The Head Agreement established as a result of this Request will constitute a standing offer, whereby Customers can place Orders on an ‘as required’ basis during the Term. Whilst this Request covers the requirement for EMHS, the contract resulting from this process may be extended to cover additional purchases from other Health Service Providers (HSPs) during the Term of the Head Agreement. | WA-TSPFH23003 | South Metropolitan Health Service (SMHS) requires the supply, delivery, installation, and commissioning of slit lamps, with imaging systems, examining consoles and patient chairs for Fremantle Hospital (FH). The Contract Authority/Customer may require additional slit lamps, examining consoles and patient chairs throughout the term of the Head Agreement. The Head Agreement established as a result of this Request will constitute a standing offer, whereby Customers can place Orders on an ?as required? basis during the Term.
