| WA-MHC118 | The purpose of the service is to provide comprehensive, culturally appropriate and flexible supports based upon the individual needs of people with a mental illness, their families and/or carers. Individual needs will be determined based upon the development and implementation of individual plans (shared between providers where possible) that are person centred, holistic with a strong focus on enhancing social inclusion, recovery and the capacity for people to achieve their desired goals and live in their community. Plans will incorporate elements of both formal and informal support and should not be limited to what a specific service can provide. Individual plans will be required to be reviewed with involvement of all key stakeholders at a minimum of every 12 months; however this should be based upon the circumstances of each individual and may require more regular reviews. Partnership arrangements with other community organisations, mental health services and any other key stakeholders should be developed and formalised (where possible) to support a holistic approach to meeting individual needs. | Mental Health Commission | $769,127 |
